A comprehensive individualized care plan, based on client needs or family requests
End of Life Care
Provide compassionate guidance and support to clients and their families coping with end of life care and challenges
Counsel, educate, and assist with the emotional needs of clients and families
Provide Education
Provide insight to family members about diagnoses, medications, disease processes, care options, and other helpful information to guide them through the many aspects of the aging process

Review medications, assess the client’s ability to take medication as prescribed, and provide medication management
